If your insurance co-pay is higher than our sliding scale fee charges and your family size and income level meet the criteria, you would qualify for the sliding scale fee price. See chart or fill out this form.

No one will be denied access to services due to inability to pay. There is a discounted/sliding fee schedule available for both insured and uninsured patients based on family size and income level. (See chart or fill out this form.)
To see if you qualify for the sliding scale, please bring one of the following: a copy of your most recent FILED taxes, the most recent 30 days of paystubs for the ENTIRE household, unemployment information, or food stamps letters.
