Community Health
Worker - Bilingual
Help us deliver care that is accessible, high quality and centered on every patient. The Quality Director leads our quality improvement, patient safety and compliance programs and turns data into better outcomes for the people we serve.
Psychiatric Consultant CoCM
Join our integrated care team and help bring whole-person care to our patients. In our Collaborative Care model, the psychiatric consultant advises our primary care providers and care managers so patients get the right support close to home.
Licensed Practical Nurse
Be a key part of our care team. Our LPNs support providers, care for patients from intake through follow-up, and help patients understand and manage their health.
Quality Director
