Patient Rights
Patients have the right to:
Respect and Dignity: Receive care in a safe, supportive, and culturally sensitive environment.
Privacy and Confidentiality: Have medical records and personal information kept confidential in accordance with HIPAA.
Access to Care: Receive timely and appropriate medical and behavioral health services.
Informed Decisions: Be informed about diagnoses, treatment options, potential risks, and expected outcomes.
Refusal of Treatment: Accept or refuse medical treatment in accordance with applicable law.
Communication: Receive clear explanations of care and information in a language or format they understand.
Continuity of Care: Receive coordinated care and referrals to specialists when needed.
Financial Transparency: Understand costs of care, available financial assistance, and billing procedures.
Grievance Process: Voice concerns, complaints, or suggestions without fear of retaliation.
Patient Responsibilities
Patients are expected to:
Provide Accurate Information: Give complete and accurate health history and personal information.
Follow Care Plans: Participate actively in treatment plans and follow instructions from healthcare providers.
Respect Staff and Others: Treat staff, volunteers, and other patients with courtesy and respect.
Appointments: Arrive on time for scheduled visits and notify the clinic in advance of cancellations.
Financial Responsibility: Understand billing procedures and cooperate in fulfilling financial obligations.
Health & Safety: Follow safety guidelines and clinic policies to ensure a safe environment for all.
