Mercy & Truth Healthcare Ministry is committed to protecting the privacy and confidentiality of patients’ health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
Information Protection
All personal health information (PHI) collected during the course of care is kept confidential and secure. This includes medical records, treatment plans, and any other health-related information. Staff are trained to handle PHI responsibly and access is restricted to those directly involved in patient care or administrative functions required for treatment, payment, or healthcare operations.
Patient Rights
Patients have the right to:
Review and request a copy of their medical records
Request corrections to their health information
Receive an account of disclosures of their PHI
Request restrictions on certain uses or disclosures of their PHI
Request confidential communications
Use and Disclosure of PHI
PHI may be used or disclosed for purposes of treatment, payment, and healthcare operations. Any other uses require written authorization from the patient, except as required by law.
Safeguards
Mercy & Truth implements administrative, technical, and physical safeguards to protect PHI from unauthorized access, use, or disclosure.
