If you get a call from ChartSpan on behalf of Mercy & Truth, it is a real call. ChartSpan is our trusted partner, and their care team is reaching out to eligible patients to share this program. If you ever have questions, call our office and we’ll be glad to confirm.

Who Qualifies.
Medicare patients living with two or more chronic conditions, such as diabetes, high blood pressure, heart disease, COPD or arthritis.
