First Time Patients
First time patients need to show up 15 minutes in advance to fill out paper work. A sliding scale fee is applicable for those who qualify.

To see if you are eligible for the sliding scale please bring a copy of your most recent financial records or at least two pay stubs to prove your income level.  
 
Based on our sliding fee discount program, the nominal charge for your visit could range between $30-$50.
