First Time Patients
First time patients need to show up 15 minutes in advance to fill out paper work. A sliding scale fee is applicable for those who qualify.

To see if you are eligible for the sliding scale please bring a copy of your most recent financial records or at least two pay stubs to prove your income level.

​Based on our sliding fee discount program, the nominal charge for your visit could range between $30-$50.
