Community Health Workers
A Community Health Worker (CHW) is a trusted member of the community and our clinic staff that understands the issues of our neighborhoods. They can help improve health by finding solutions and educating clients to meet their personal goals.
You will benefit from a Community Health Worker if you need assistance with:
• improving your health but don’t know where to start
• trying to overcome a difficult situation
• any mental health concerns
• getting medications or understanding treatment plan
• housing, food, childcare, transportation, employment, Medicaid application, etc.
• mobility and require a home visit
• translation while visiting one of our providers
• a recent post-ER visit or hospital stay OR
• visiting our clinic
Our Community Health Workers specialize in helping clients with:
• High blood pressure, diabetes, and other difficult health conditions
• Resource coordination (ex: housing, utilities, transportation, healthcare needs)
• Goal setting
• Health coaching through our healthy lifestyle programs
• Translator services – We have CHWs who speak English, Spanish, Swahili, and Nepalese.
